Fat chance on this one.Carol v2 Kade Engine by imakegoodsuf2. Since the only permanent fix can only come from Bethesda. In truth this is just something I have learned to live with. Settlers mostly, but I think it is responsible for a few quest NPC's to have disfigured faces. Mods like Unique NPC's can, and does make some permanently have the black face bug. Most never get the black face bug, Piper get's it regularly.
Same goes for the companion appearance changers I have. So does doing anything that makes the game reload, like fast travel. Quitting, and reloading works for me every time. The game just dose not like custom faces. I do use looks menu, and I do think it is part of the reason it happens. if the game load's to slow, or if I have to reload a save during playing, mostly due to getting killed. I don't use any of the three mod's mentioned, as I don't really need them during normal play. I do not think there is a permanent fix for this, as it more of a game engine bug over anything else.
